Consumption by indication
General dosage
Adults: 10-40mg (IV or IM) and if needed, dosing will be given to the patient for an intravenous dose of 1 to 2 minutes.
Children: 139-835mcg / kg every 12 to 24 hours (IV or IM)
High-dose pulsed therapy (for example, lupus nephritis)
Adults and children: 30mg / kg for at least 20 to 30 minutes of infusion (IV). If needed, it can be repeated every 4 to 6 hours. Duration of treatment, maximum 48 to 72 hours.
Acute spinal cord injury
Adults: 30mg / kg are injected within 15 minutes (IV), followed by infusion of 5 / 4mg / kg / hour continuously for 22 hours after 45 min.
Children: Same as adults
Note: Only suxinate form can be injected intravenously.
Contraindications and precautions
Methylprednisolone is contraindicated in patients who are susceptible to that drug for people with systemic and fungal infections and for premature infants.
It is not recommended during the course of lactation.
It is also recommended for the following diseases: gastrointestinal ulcer, kidney disease, hypertension, osteoporosis, diabetes mellitus, hypothyroidism, cirrhosis, ulcerative colitis, thromboembolic disorders, seizure, myasthenia gravis, heart failure, tuberculosis, ocular hypertension, emotional instability and Psychotic behaviors and also for pregnant women.
Important drug interactions:
Drug: The use of macrolide antibiotics reduces methylprednisolone purification and may increase its effect.
Medical attention - nursing
1) Monitor the weight, blood pressure, serum electrolytes, and the patient's sleep patterns. At first, the euphoria may prevent the patient from sleeping. But after one to three weeks, the patient gets used to the medicine. 2) Note that only Sodium Succinate Methyl Prednisolone Sodium Solution can be injected intravenously, and acetate salt should only be injected via muscle. 3) The intravenous injection should be at least 1 minute long. To treat shock, when a high dose of the drug is injected, the duration of the intravenous injection should be at least 10 minutes in order to prevent arrhythmias and collapse. 4) Prepared solutions can be used up to 48 hours. 5) Take intramuscular injection of acetate salt deeply. When a high dose of this drug is injected into a muscle at once, skin atrophy may occur. Lowering the injection into smaller amounts at different points may reduce the chance of atrophy. Shake the injection thoroughly before each injection. 6) Avoid subcutaneous injection. Skin atrophy or sterile abscess may be created. 7) Injection of this drug in the cerebrospinal fluid is prohibited. 8. Acetate salts are not suitable for the treatment of acute problems. 9) Always try to use the lowest possible dose. 10) In the long term gradually reduce the dose. 11) In general, adrenal suppression occurs less with drugs such as methylprednisolone, prednisolone or prednisone.
